A Calcium Score of Zero Doesn't Erase a High LDL: Why I Hesitate to Skip the Statin

It’s a conversation I have often in my Encinitas office. A patient sits across from me with two numbers that seem to argue with each other. Say her LDL cholesterol is 165 and her coronary calcium score is zero. On the scan, her arteries look clean. On the cholesterol panel, she’s carrying a lot of the exact particle that builds plaque. She wants to know the thing I keep chewing on after she leaves the room. Is that zero a green light to skip the cholesterol medicine, or is it a quiet clock that hasn’t started ticking loudly yet?

That tidy version of the advice makes me uneasy, and I want to walk you through why. A coronary calcium score is a quick, low-dose CT scan that looks for hardened calcium in the walls of the heart’s arteries. A score of zero means the scan found no calcified plaque. LDL cholesterol is the particle that drives plaque over years. When those two readings point in opposite directions, the calcium score tends to win the argument in a busy clinic. Sometimes that’s the wrong call.

What the guideline actually says

The guideline is built on real evidence, so start there. The 2026 ACC/AHA dyslipidemia guideline says that for adults at intermediate or lower risk, a calcium score of zero makes it reasonable to hold off on a statin and reassess later. That leans on a study of more than thirteen thousand patients that found no clear statin benefit, on average, over about ten years in people whose calcium score was zero. On its face the logic holds. No calcified plaque, very low near-term event rate, so why start a lifelong medication today?

That same guideline names the people this doesn’t apply to. A zero score shouldn’t talk you out of treatment if you have familial hypercholesterolemia (an inherited condition that keeps LDL very high from birth), if your LDL is 190 or above on its own, if you have diabetes, if you smoke, or if you have a strong family history of early heart disease. It also asks that anyone who defers get the scan repeated later, because the picture changes with time. Those carve-outs are the guideline admitting that a zero isn’t the whole story.

Where I’m willing to wait

I’ll tell a patient plainly that the current guidelines may support waiting, and I’ll use a zero score to ease off the gas. Picture a 68-year-old with an LDL of 135, normal blood pressure, no diabetes, no tobacco, no family history of early heart attacks, and a calcium score of zero. Her ten-year risk on paper sits right at the border. If she’s strongly averse to starting a medication, I’m glad to take the more permissive road with her. We lead with diet and exercise, and we plan a real reassessment and a repeat scan in a few years. The guideline supports that, and for the right person it’s a fair place to land.

I’ll also be honest about where I stand while we make that call. My bias runs toward the European approach, which favors treating the cholesterol over waiting. So even when I agree to hold, I want it to be a shared decision we made with eyes open, not a question a zero score closed on its own.

Calcium is a late sign, not an early one

Calcium is one of the last things to show up in a plaque’s life story. A plaque starts as soft, cholesterol-rich material in the artery wall. It can grow, and it can rupture and cause a heart attack, before the body ever lays down the calcium a scan detects. So a score of zero doesn’t mean there’s no plaque. It means there’s no calcified plaque yet.

Registry data back this up. In the Western Denmark Heart Registry, researchers looked at nearly 24,000 people who had a calcium score of zero and then went on to a CT angiogram, which can see the soft plaque a calcium scan misses. About one in nine of them, roughly 11 percent, already had non-calcified plaque behind the reassuring zero. The higher the LDL, the higher the odds of finding that hidden plaque, and the effect was strongest in people 45 and younger. In that same group, a higher LDL still tracked with more coronary events over time even when the calcium score read zero.

That first number is the one I sit with. Soft plaque is exactly the kind that ruptures, so a real share of heart attacks strike people whose calcium score was zero. The scan is hunting for the scar of an old, stabilized process. Meanwhile the dangerous plaque is usually the young, soft one that hasn’t scarred over.

A zero score comes with a warranty, and it’s shorter than it sounds

A calcium score is a snapshot, not a forecast. When researchers followed people with a zero score in the Multi-Ethnic Study of Atherosclerosis, plenty of them didn’t stay at zero. Roughly a quarter developed detectable calcium within a few years, and how fast that happened depended on who they were. Lower-risk people held their zero longer. People with diabetes converted the fastest.

So when I tell a 45-year-old with a high LDL that her zero score buys her a pass, I have to ask how long that pass stays good. If a quarter of people like her grow calcium within three to five years, the zero is a snapshot of today, not a forecast of the decade in front of her. And she has four decades in front of her.

Five years of calm is not a lifetime of safety

Those short studies have a blind spot. They measure about five years, and over five years a high LDL does add little on top of a clean scan. Cholesterol works slowly, though. It’s the total exposure over decades that builds plaque and eventually destabilizes it.

Watch what happens when the clock runs longer. Young adults followed for 25 years who carried a high LDL, even the ones who started with a zero calcium score, ended up with several times the cardiovascular risk of peers who kept their LDL low. Genetics tell the same story. People born with variants that hold their LDL naturally low get far more protection over a lifetime than a statin started in middle age delivers in a trial, because the low number was there the whole time. A five-year study can’t see any of that.

A 50-year-old sits right in the trap this creates. Her scan is clean, her five-year risk is low, and the guideline gives us cover to wait. Meanwhile the LDL keeps working, and the years she’d benefit most from lowering it are the years we’re choosing to spend watching.

That thirteen-thousand-patient study cuts the other way

The “no benefit at zero” claim rests mostly on that one cohort of thirteen thousand patients, and it pays to read past the headline. The primary analysis showed no statin benefit at a zero score. Then the authors looked at how long people actually stayed on the drug. Among the patients who took a statin for more than half of the follow-up, the ones who truly used it, major cardiovascular events dropped by about a third even with a calcium score of zero. The people who took the statin did better, and their event rates came out lower. That’s the opposite of the message the headline carries.

They said as much in the paper. A modest benefit of sustained statin therapy may still exist at a zero score, and treatment can still be warranted when other risk factors are present. The study quoted to justify waiting carries its own argument for treating.

Treating has little downside, and there’s no LDL that’s too low

Weigh what each choice actually costs. A statin is cheap, generic, and one of the best-studied drugs in all of medicine. Real side effects happen, and they’re uncommon. The muscle aches people fear turn out, in blinded trials, to show up about as often on a placebo pill, which I’ve written about in the statin muscle-pain piece. On the other side of the ledger sits silent plaque.

The old worry that you can push LDL too low hasn’t held up either. When the PCSK9 drugs drove LDL down into the teens and twenties, the people who got there kept gaining benefit, with no floor where it reversed and no new safety signal that tracked with the low number. On imaging, aggressive lowering did something a calcium score can’t promise. It shrank plaque. In one trial that scanned the inside of the coronary arteries, adding a strong LDL-lowering drug on top of a statin regressed the plaque in most patients.

Line those up and the choice tilts hard. Leave a high LDL alone and the likely result is subclinical atherosclerosis, plaque you can’t feel and a calcium score can’t yet see, forming quietly for years. Treat the LDL to a lower goal and you make that outcome less likely. The downside of acting is a cheap daily pill. The downside of waiting is the disease itself, taking shape where nobody’s looking.

Residual risk, and why I hate leaving the best lever unused

Even when we do everything right, some cardiovascular risk stays behind. We call it residual risk, the part left over after we’ve treated what we can. Some of it runs through channels we barely control yet, like inflammation or an inherited particle I’ll get to in a minute. LDL is the exception. It’s the most controllable causal driver we have, with the deepest evidence behind lowering it. Leaving a high LDL untreated hands back the one lever with the best odds of pushing that residual number down.

The zero score has its own version of the problem. When researchers applied the newer PREVENT risk calculator, the one the 2026 guideline endorses, to people who already met the bar for a statin, the ones with a zero score still ran close to six cardiovascular events per thousand patient-years. That’s not a trivial rate. It sits above the level where most of us would offer treatment. The authors put it bluntly. A zero score shouldn’t, on its own, be used to withhold a statin from someone who already qualifies.

Lipoprotein(a) rewrites the math

One number changes how I read a zero score more than almost any other. Lipoprotein(a), or Lp(a), is a cholesterol particle you inherit. About one in five people carry it at a high level, and it drives heart attacks and valve disease earlier than the standard panel predicts. Statins don’t lower it. A regular cholesterol test won’t flag it unless you order it on purpose.

Here’s what that does to the calcium conversation. Someone with a high Lp(a) is stacking up atherosclerotic risk through a route the calcium score is blind to, and the tools we have to fight back all run through aggressive LDL and apoB lowering. So a zero calcium score doesn’t settle me down the way it otherwise might when the Lp(a) is high. I favor treating the LDL hard in those patients, calcium or no calcium, and driving the number well under a standard target. The clean scan describes today. The Lp(a) describes a lifetime.

Europe doesn’t play it this way

American guidelines lean on the calcium score to decide who gets to wait. The European Society of Cardiology and European Atherosclerosis Society go a different direction. They don’t downgrade risk or defer treatment because a calcium score reads zero, and their LDL targets run lower and harder than ours. For their highest-risk patients they aim for an LDL under 55 mg/dL along with at least a halving from baseline. For high-risk patients, under 70. They push to start lowering early rather than watch and wait. Same ocean of evidence, a more aggressive read of it. When two expert bodies study the same data and one says “reasonable to defer” while the other says “treat lower, sooner,” that gap alone should give anyone pause before treating a zero as permission to do nothing.

I’m not the only one uneasy about this

I’m not out on a limb here, and you should hear the range, including the people who disagree with me. James Stein, a preventive cardiologist at the University of Wisconsin, wrote that the “power of zero” has become a myth, for the reason I gave earlier. Calcification is a lagging marker, so a zero can hand false comfort to a younger person with a high LDL. Ann Marie Navar at UT Southwestern put it plainly when a study like this hit the news. When we prevent heart disease, she said, we’re thinking about a lifetime, not four years. Raul Santos, in an editorial on the same data, singled out younger patients, whose lifetime risk the short studies can’t capture.

The other side has serious people too, and they’ve earned their position. Matthew Budoff, one of the pioneers of calcium scanning, argues that starting a statin in someone with no calcium may be close to futile, because LDL becomes a strong predictor mainly once plaque exists. Martin Mortensen, who led much of the Danish work, urges the same caution I do, pointed the other way. He warns against stretching the reassuring short-term data onto young people, familial hypercholesterolemia, or twenty-year horizons. Read him closely and he’s guarding against the exact overreach I’m worried about.

How I actually use a zero score in clinic

Here’s where it lands, patient by patient. If you’re older, your risk is intermediate, and you carry none of the red flags, waiting is defensible, and I’ll say so. When a patient in that spot is set against starting a pill, I’ll take the permissive road with them. We lead with lifestyle, plan a repeat scan, and set an honest reassessment a few years out. I’ll also disclose my own bias, which follows the European guidelines more closely and favors treating rather than waiting. So if we wait, we wait together with eyes open, not because the zero settled it.

If you’re younger and your LDL is high, I lean toward treating the cholesterol whether or not calcium has shown up yet, because you have the most lifetime exposure to prevent and the most to gain. If you have familial hypercholesterolemia, an LDL of 190 or higher, diabetes, a smoking history, a strong family history of early heart disease, or a high lipoprotein(a), a zero score doesn’t change my recommendation. Those are the patients the guideline itself carves out.

When the decision is genuinely close, I’ll sometimes order a CT angiogram instead of a plain calcium score, because it shows the soft plaque a calcium scan misses. An artery that’s actually clean, not just uncalcified, reassures me far more than a zero on its own. And treating cholesterol doesn’t mean jumping to the strongest drug on day one. It usually starts with lifestyle, then a statin, with add-ons like ezetimibe and, for the right person, PCSK9 inhibitors when we need to go further.

The bottom line

A calcium score of zero answers a narrow question well. It tells you whether you have hardened plaque today and how low your risk runs over the next few years. It says nothing about whether a high LDL will build new plaque over the next few decades, which for a lot of people is the question worth answering. Use the zero for what it’s worth, and don’t let it argue you out of protecting a heart with forty more years of work ahead of it. If your cholesterol is high and your scan is clean, that’s not a closed case. It’s the start of a real conversation with your own doctor about your age, your family, your Lp(a), and how much runway you’re trying to protect.

Common questions

If my calcium score is zero, why would I still need a statin?

Because calcium shows up late. A zero means no hardened plaque today, and soft plaque that hasn’t calcified yet can still be there, especially if your LDL is high. If you’re younger or carry other risk factors, the cholesterol is worth treating for the decades ahead, not just the next few years.

Doesn’t a zero score mean my arteries are clean?

Not exactly. It means they’re free of calcified plaque. On CT angiography, about one in nine people with a zero score already have soft plaque. And a real share of heart attacks happen in people whose calcium score was zero, because soft plaque is the kind that tends to rupture.

I’m in my 40s with a high LDL and a zero score. What would you do?

In most cases I’d treat the cholesterol. Your scan is reassuring for the short term, and your best window to prevent a lifetime of plaque buildup is right now. I’d look at your family history, your lipoprotein(a), and your other numbers, then usually start with lifestyle and a statin rather than wait.

My Lp(a) is high but my calcium score is zero. Does that change anything?

Quite a bit, for me. High Lp(a) drives risk through a path the calcium score can’t see, and statins don’t lower it. The lever we do have is lowering your LDL aggressively. A zero score doesn’t reassure me much when Lp(a) is high, so I usually treat the cholesterol hard anyway and aim for a lower goal.

How often should I repeat the scan if we decide to wait?

Every three to five years is a reasonable rhythm for most people who defer, and sooner if you have diabetes or your risk is climbing. A quarter of people with a zero score develop calcium within a few years, so the follow-up scan is part of the plan, not an afterthought.

Is a CT angiogram better than a calcium score for this?

For the soft-plaque question, yes, it can see disease a calcium score misses. It uses contrast dye and more radiation, so I reserve it for cases where a plain calcium score leaves the decision genuinely unsettled. For most people the calcium score plus a good history is enough.
